Drenagem Percutânea da Vesícula Biliar por Via Trans-hepática para Colecistite Aguda Durante a Quimioterapia: Série de Casos

Resumo

Os clínicos podem enfrentar dificuldades no tratamento da colecistite aguda em pacientes que estão em quimioterapia. Este estudo teve como objetivo revisar a viabilidade de continuar a quimioterapia enquanto um dreno biliar permanece instalado e relatar nossa experiência. Examinamos retrospectivamente os prontuários de 17 pacientes submetidos à drenagem percutânea da vesícula biliar por via trans-hepática (PTGBD) para colecistite aguda. A coorte incluiu nove casos de câncer gastrointestinal e oito de outras neoplasias malignas. No total, a quimioterapia foi retomada em 12 casos após a PTGBD. Esses pacientes foram divididos em três grupos com base nas circunstâncias em que a quimioterapia foi reiniciada. A quimioterapia foi realizada com o dreno ainda posicionado em sete pacientes (14,33 ± 6,94 dias até o início da quimioterapia após a drenagem, grupo A), após colecistectomia em três (84 ± 56 dias, grupo B) e após a retirada do dreno (56 dias, grupo C) em dois pacientes. A administração precoce da quimioterapia só foi possível no grupo A. Em alguns momentos, os clínicos precisam tomar decisões complexas em relação ao tratamento da colecistite aguda, que não podem ser feitas apenas com base em diretrizes. Com controle rigoroso, a quimioterapia pode ser retomada com segurança, mesmo com o dreno ainda posicionado, e a PTGBD pode ser cuidadosamente considerada para esses pacientes. Esses grupos foram determinados retrospectivamente com base na evolução do tratamento.
