Traumatismo Cranioencefálico grave associado a Politrauma e Complicações Renais secundária a Rabdomiólise: um relato de caso e manejo clínico inicial

Abstract
O Traumatismo Cranioencefálico (TCE) é causado por qualquer trauma que leve à lesão de estruturas do crânio, desde o couro cabeludo até o tecido cerebral. Essas lesões, quando graves levam a comprometimento estrutural e funcional de intensidade variável, podendo levar à morte ou sequelas importantes ao indivíduo acometido. Apresentamos o relato de paciente internado em uma UTI de Vitória - ES com TCE grave decorrente de um politrauma que evoluiu com rabdomiólise, injúria renal aguda e choque hipovolêmico. Relatamos um caso de paciente do sexo masculino, 25 anos, vítima de colisão moto x moto, apresentou em Tomografia Computadorizada (TC) de crânio fratura frontal esquerda com extensão até a porção escamosa do osso temporal ipsilateral e asa maior do esfenoide, hematoma epidural fronto-temporal esquerdo, hematoma periorbitário à esquerda e achado indicativo de hemorragia subaracnoidea. Os exames realizados subsidiaram o diagnóstico de injúria renal aguda e rabdomiólise, enquanto a monitorização hemodinâmica realizada permitiu o diagnóstico de choque hipovolêmico. O TCE é uma condição clínica que frequentemente expõe o paciente a um maior número de complicações, como a insuficiência renal, lesão renal aguda e distúrbios metabólicos. Dessa forma, a terapêutica precoce e adequada de tais desordens configura-se essencial para um bom prognóstico.
