Abstract

A mielite transversa aguda é uma complicação extremamente rara, que se apresenta com sintomas sensório-motores e distúrbios de membros inferiores, além de retenção urinária. O presente estudo descreve um caso de mielite transversa aguda pós-infecciosa causada pelo vírus da dengue em um homem de 35 anos de idade. As manifestações neurológicas da dengue incluem encefalite, mielite e meningite, e estão presentes em cerca de 4% a 9.3% dos pacientes. No nosso relato o paciente além de apresentar sintomas clássicos da dengue apresentou alteração de sensibilidade cutânea. Posteriormente, desenvolveu parestesia em membros inferiores, disúria, oligúria e alodinia cutânea abaixo do dermátomo T7-T8, levando à internação. Para diagnóstico foi utilizada ressonância magnética de neuro-eixo que confirmou a suspeita. O tratamento foi iniciado com pulsoterapia com metilprednisolona 1g/dia durante 3 dias, levando a uma melhora significativa dos sintomas sensitivos e do quadro em sela. A mielite transversa pós-infecciosa é uma complicação rara da dengue, e deve ser considerada no diagnóstico diferencial de manifestações neurológicas associadas a essa arbovirose. O tratamento com corticoterapia pode ser uma opção eficaz para os pacientes com essa condição.
